Starting a family or welcoming another child into your family is a wonderful experience and truly a gift. Along with the beauty and joy of pregnancy, delivery and then coming home, there are bound to be times when you have questions about how to care for your baby and family.
We are here for you and your family.

We are excited and honored that you are considering us for your child’s care. It is always special for us to be invited by new parents to join in the care of a family member. The continuity of care and the amazement of watching a baby grow and develop into a wonderful child is one of the primary reasons that we all feel privileged to work in pediatrics.
First Appointment
Please plan to arrive 15 minutes prior to your scheduled appointment time to fill out a patient information sheet and familiarize yourself with our billing procedures. Please bring any paperwork from the hospital, including hearing screening results and vaccine record. You will also need to bring your current insurance card. We will be happy to bill your insurance company if we are contracted network providers.
To make your appointment go as smoothly as possible, please contact your insurance company prior to your visit to understand your benefits. Pediatric Health Associates has no way to track your benefits, so it is important for you to know if you are close to meeting your maximum benefits.
Some key questions to ask are:
- Do I have well coverage?
- What are the yearly limits?
- Do I have immunization coverage?
- What are the yearly limits?
- How much is my co-pay and deductible?
- What lab can I go to? Do I need a referral?
There are multiple components to every office visit. The physicians/nurse practitioner's exam, the administration of vaccines, the cost of the vaccines, minor procedures and lab work are all possible charges you may incur to name just a few. Please note: If you have limited well or immunization coverage, you may obtain most vaccines at your local health department for a nominal fee. If you do not have ANY well child or immunization coverage, please ask about our Vaccines for Children program. Lastly, please be advised that a parent or legal guardian must accompany a child at each visit and that our appointment cancellation policy is 24 hours prior to visit.
Please Bring The Following To Your First Appointment:
- Any paperwork from the hospital, including hearing screen results and immunization records.
- Any available insurance information (often only the parent’s insurance is available at the first visit).
- Plenty of diapers and an extra set of clothes.
- Please complete the new patient forms prior to the visit to help make the appointment as smooth as possible.
